Nottinghamshire Integrated Wellbeing Service (IWS) overview

ABL Health and Nottinghamshire County Council are partnering to deliver and Integrated Wellbeing Service across Nottinghamshire.

This transformative service will play a central role in system change, bringing previously separate lifestyle behaviour change functions together into one service.

It seeks to work collaboratively with communities to support the transformation of services for the local population.

The service will provide integrated behaviour support for smoking cessation, weight management, alcohol reduction, physical activity and falls prevention, all embedded in sustainable behaviour change and positive wellbeing.

Role Purpose

As a Health Promotion Officer for the Your Health Notts Integrated Wellbeing Service, you will work alongside communities to improve health outcomes through education, engagement and community development.

Working across targeted former mining communities, you will build trusted relationships with residents, voluntary organisations, community groups, health professionals and partner agencies to improve awareness of local health services and support people to access appropriate wellbeing interventions.

You will support individuals and communities to increase their understanding of health and wellbeing, helping to build confidence, improve health literacy and encourage positive lifestyle changes.

As part of your professional development, you will complete the RSPH Level 2 Award in Understanding Health Improvement (if not already achieved), equipping you with the knowledge and skills to promote health improvement within local communities.

Using a strengths-based, asset-based community development approach, you will identify local assets, listen to community priorities and work collaboratively with local people to reduce health inequalities and improve access to preventative health services.

This is a varied and rewarding role requiring flexibility, initiative and excellent communication skills.

No two days will be the same, with opportunities to engage with local communities, attend community events, support local projects and work alongside colleagues across the Integrated Wellbeing Service.

You will receive support from ABL Health’s experienced multidisciplinary team, including health professionals, community engagement specialists, service development, business intelligence, governance, finance and operational teams.

Key Duties and Responsibilities

Working under the line management of the System Integration Lead, you will play a key role in delivering and developing community-focused health promotion initiatives across Nottinghamshire.

You will promote health and wellbeing within targeted former mining communities, helping to reduce health inequalities and improve health outcomes by building trusted relationships with residents, community groups, voluntary and faith organisations, local authorities, Primary Care Networks, GP practices and other partners.

Acting as a trusted link between local communities and health services, you will support individuals to access appropriate services, promote the Integrated Wellbeing Service through outreach, engagement events and awareness campaigns, and encourage referrals into smoking cessation, weight management, alcohol reduction, physical activity and falls prevention programmes.

You will support individuals and communities to improve health literacy, confidence and understanding of healthy lifestyle behaviours, while working collaboratively with partners to identify local health priorities, build community capacity and co-produce effective health promotion initiatives.

The role will involve attending community events, local forums, partnership meetings and networking opportunities, working closely with colleagues across the Integrated Wellbeing Service to provide seamless support and appropriate referrals.

You will collect monitoring information, community feedback and outcome data to support evaluation and continuous improvement, maintain accurate records, contribute to reporting requirements, and support communication and engagement activities, including awareness campaigns, promotional materials and social media where appropriate.

You will be encouraged to demonstrate creativity and innovation in developing new approaches to engaging communities and reducing health inequalities, work flexibly across Nottinghamshire including occasional evening or weekend events, and support colleagues across the wider service to ensure high-quality service delivery.

As part of your induction and ongoing professional development, you will also be expected to undertake the RSPH Level 2 Award in Understanding Health Improvement if you have not already achieved this qualification.

Skills and Competencies Required

  • Experience of working within community health, public health, health improvement, social prescribing, community development or a related field.
  • Experience of engaging effectively with local communities and building trusted relationships.
  • Understanding of health inequalities and the wider determinants of health.
  • Knowledge of community engagement approaches and asset-based community development principles.
  • Willingness to undertake and successfully complete the RSPH Level 2 Award in Understanding Health Improvement as part of your induction and ongoing professional development (or already hold an equivalent qualification).
  • Excellent communication, interpersonal and relationship-building skills.
  • Ability to communicate effectively with people from a wide range of backgrounds and adapt communication to meet individual needs.
  • Ability to motivate, empower and support individuals to make positive changes to their health and wellbeing.
  • Experience of working collaboratively with partner organisations and multidisciplinary teams.
  • Excellent organisational skills with the ability to manage competing priorities.
  • Ability to work independently while contributing positively within a supportive team environment.
  • Confident using IT systems and maintaining accurate records.
