This is a highly rewarding role that offers the opportunity to make a meaningful difference to peoples lives. The Specialist Stop Smoking Practitioner will support the delivery of evidence-based stop smoking services aimed at reducing health inequalities across the borough of Sefton. The role will include delivering stop smoking and tobacco dependence interventions throughout multiple venues across Sefton including Community centres, GP practices and workplaces. The postholder must be able to work flexibly, independently, and collaboratively as part of a team and be willing to work one evening per week to meet the needs of the service.

Part of this role will also involve occasional work with hospital settings. This will include reviewing and updating referral pathways to make sure patients are referred to the service promptly when they are discharged from hospital.

Training will be given to ensure staff / applicants are equipped to deliver effective services, Working under the line management of the Head of Service, you will play a key role in delivering and developing high-quality smoking cessation services across the local community. You will work collaboratively with partner organisations and services to promote the service, increase referrals, and embed smoking cessation into routine client and patient care. You will motivate and empower individuals to make positive, sustainable changes, providing evidence-based stop smoking interventions through a range of delivery methods, including one-to-one appointments, drop-in and group sessions, telephone and online support, in line with the NCSCT Standard Treatment Programme. You will provide evidence-based education and ongoing motivational support, work with clients to set realistic goals, and lead peer support sessions for successful quitters to help prevent relapse. The role will also involve interpreting health, equality and service data to understand local needs and inform service delivery, while ensuring a high-quality, efficient and cost-effective service across diverse community settings. You will be responsible for maintaining accurate, up-to-date client and service records in line with ABL Health documentation policies and will need to manage competing priorities effectively, using your own initiative and professional judgement to meet agreed timescales.
