Working with CYP emotional needs across all elements of the iTHRIVE model. This diverse and exciting role delivers a range of interventions to CYP (children and young people) at various stages of their emotional recovery and safety needs.
Working as part of a multi-disciplinary community team, you will work on a rotational basis across the community day & educational provisions as well as the urgent need Haven365 Service including working across days, evenings, weekends & bank holidays.
The successful candidate will provide direct emotional support to children and young people (CYP) across both the Haven evening service and Hubs day services, supporting their mental health needs within the Getting Advice / Getting Help elements of the iTHRIVE model.
In the Haven service, you will act as lead support, providing immediate, person-centred care following referrals from partners such as Police, Ambulance, ED and CAMHS, conducting rapid safety assessments, de-escalating challenging behaviours, and coordinating ongoing safety plans, including safe returns home or emergency accommodation.
You will develop and implement follow-up support plans, liaise with families and referral partners, maintain accurate records, and support the Service Lead in managing the service, including supervising a small team, coordinating rotas, assisting with recruitment, and providing on-call support.
In the Hubs, you will deliver emotional wellbeing support in community and educational settings, developing individualised support plans, working with families, schools, healthcare providers and community organisations to coordinate care, advocate for CYPs mental health, respond to safeguarding concerns, and promote awareness of available services.
The role also involves participating in training, supervision, and professional development, contributing to service development, and supporting the delivery of high-quality, child-centred care across all settings.
