An exciting opportunity has arisen to join an innovative service redesign in South Aberdeenshire! We are seeking a Care Manager to become an integral part of our newly developed Enhanced Dementia Assessment Team - a dynamic, multi-disciplinary service designed to deliver person-centred care and creative support for individuals living with dementia.

Based at Glen O'Dee Hospital in Banchory, this role will also cover the wider Deeside, Kincardine & Mearns areas, offering you the chance to work across diverse communities and make a real difference where it matters most. This is a part-time position (18 hours per week), and we are committed to supporting a flexible working pattern that suits both the service and the successful candidate.

You will collaborate closely with skilled professionals from health and social care, contributing to assessments, care planning, and interventions that enhance quality of life for individuals and their families. There is a responsibility for the post holder to demonstrate a commitment to quality service delivery through continuous improvement for the benefit of the service and the organisation.

Requirements:

  • This post is regulated work with children and/or protected adults under the Protection of Vulnerable Groups (Scotland) Act 2007.
  • The preferred candidate will be required to join the PVG Scheme or undergo a PVG Scheme update check.
  • Where an individual has spent a continuous period of 3 months or more out with the UK in the last 5 years, an Overseas Criminal Record Check will be required.
  • A confirmed offer of employment and commencement in the post will be subject to the outcome of both these pre-employment checks being deemed satisfactory.
  • This post has a minimum requirement of two references, one of these must be your current or most recent employer.
