Our organisation exists to support anyone and everyone who is facing difficulty and we provide a wide range of life-changing services including support to people who are homeless or at risk of homelessness, have experienced abuse, are at risk of violence, are at high financial risk, struggling with mental health issues, struggling with substance addiction, social exclusion or involved in the justice system.

Responsible to: Independent Living Service Manager

Hours of work: Full-time (36.5 hours per week)

Contract: Permanent

Salary: £23,760

Location: Field-based working across all three localities of Aberdeen City

Main Responsibilities

  • Lead and participate in the provision of a high standard of person-led care and support for service users in their own homes, in line with individual care plans and risk assessments, organisational policies, national standards, codes of practice and legislation.
  • Communicate effectively with service users, families and multi-disciplinary teams, record appropriate care notes and create a safe, positive and enabling environment.

Job Activities

  • Understand the physical, emotional, cultural, and social needs of the service users, including a wide range of needs, illnesses and disabilities.
  • Provide a wide range of domiciliary care including personal and continence care, managing medication, household tasks and meal preparation as agreed in individual care plans.
  • Have an understanding of palliative and end-of-life care.
  • Have an understanding of working with individuals who may be elderly and vulnerable.
  • Work to a person-centred and enabling approach.
  • Observe and promote the service user’s choice, independence, dignity, privacy, fulfilment and other rights.
  • Actively support other Support Practitioners.
  • Undertake the tasks detailed in the service user’s care and support plan using trauma-informed practices.
  • Provide input into the care and support plans alongside the service user.
  • Use manual handling equipment safely and correctly.
  • Establish and maintain good communication and develop effective working relationships with service users, families and multi-disciplinary teams.

Essential

  • A minimum qualification of N/SVQ3 in Adult Health and Social Care, or equivalent professional qualification or equivalent professional experience and a willingness to work towards SVQ3.
  • Literacy and numeracy at a level to maintain all the appropriate paperwork and report writing.

  • Maintenance of records – Utilisation of PASS to record all information in relation to Service User Case Management.
  • Team meeting attendance.
  • Office 365 and IT Platforms.
