The role will include tasks such as:
- Helping people to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible.
- Visiting Service Users in their own homes to provide appropriate care and support as defined in the Care Plan.
- All aspects of personal care, administering medication, monitoring of vital signs, exercise and domestic tasks if needed.
- Liaising with medical professionals such as district nurses and pharmacies.
The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well-being.
Working hours are between 7am-10pm with shifts patterns of 7am-2pm and 4pm-10pm which are inclusive of alternate weekends. We offer high pay rates of £13.43ph on weekdays and £15.50ph on weekends. The hourly rate is subject to holding a NVQ level 2 or above in Health and Social Care.
