This role will be based covering Watford and surrounding areas.
Key Responsibilities
- Helping people to leave hospital as soon as is practicably possible and to safely transition back to living at home as independently as possible.
- Visiting Service Users in their own homes to provide appropriate care and support as defined in the Care Plan.
- All aspects of personal care, administering medication, monitoring of vital signs, exercise and domestic tasks if needed.
- Liaising with medical professionals such as district nurses and pharmacies.
- The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well‐being.
Working hours are between 7am‐10pm with shift patterns of 7am‐2pm and 4pm‐10pm which are inclusive of alternate weekends.
Remuneration
We offer high pay rates of £13.43ph on weekdays and £15.50ph on weekends. Hourly rate is subject to holding a NVQ level 2 or above in Health and Social Care.
