The role will include tasks such as:

  • Helping people leave hospital promptly and transition safely back to independent living at home.
  • Visiting Service Users in their homes to provide care and support as outlined in the Care Plan.
  • Providing personal care, administering medication, monitoring vital signs, assisting with exercises, and domestic tasks if needed.
  • Liaising with medical professionals such as district nurses and pharmacies.

The goal is to rehabilitate, maximize potential, and support Service Users in activities that promote independence and monitor well-being.

Working hours are between 7am-10pm, with shift patterns of 7am-2pm and 4pm-10pm, including alternate weekends. We offer competitive pay rates of £13.43 per hour on weekdays and £15.50 per hour on weekends. Note: The hourly rate is subject to holding an NVQ Level 2 or above in Health and Social Care.
