The role includes tasks such as: Helping people leave hospital promptly and safely transition back to independent living at home. Visiting Service Users in their homes to provide care and support as per the Care Plan. Providing personal care, administering medication, monitoring vital signs, assisting with exercises, and domestic tasks if needed. Liaising with medical professionals such as district nurses and pharmacists. The goal is to rehabilitate and maximize potential, supporting Service Users in activities that promote independence and monitor well-being. Working hours are between 7am-10pm, with shift patterns of 7am-2pm and 4pm-10pm, including alternate weekends. We offer competitive pay rates of £13.43 per hour on weekdays and £15.50 per hour on weekends. *Note: The hourly rate applies if you hold a NVQ Level 2 or above in Health and Social Care.
