Role
This role will cover Watford and surrounding areas.
Key Responsibilities
- Helping people to leave hospital as soon as practicably possible and to safely transition back to living at home as independently as possible.
- Visiting Service Users in their own homes to provide appropriate care and support as defined in the Care Plan.
- All aspects of personal care, administering medication, monitoring of vital signs, exercise and domestic tasks if needed.
- Liaising with medical professionals such as district nurses and pharmacies.
The aim is to rehabilitate and maximise potential, to support and enable Service Users in a range of activities to promote independent living and monitor their well‑being.
Working hours are between 07:00 and 22:00 with shift patterns of 07:00–14:00 and 16:00–22:00, which are inclusive of alternate weekends.
Pay
High pay rates of £13.43/hr on weekdays and £15.50/hr on weekends.
* The hourly rate is subject to holding an NVQ level 2 or above in Health and Social Care.
