Within the role as the Care Coordinator, you are required to manage the organisation and delivery of care. You will manage a team of care assistants to support our service users in their own home to provide person centred care. This role is more than just rota management. You will be a part of our operational team, who ensure that we provide outstanding quality care is delivered consistently to our service users.

The role:

  • Rostering and daily management of Care Staff who are based in the community
  • Working with multi-disciplinary teams and professional bodies to ensure outstanding care is delivered and to positively promote our services.
  • Complete quality calls with care staff and service users
  • Provide reports to senior management as and when required
  • Provide support to our teams as part of a rostered-on call system
  • Manage referrals from commissioners and local authority
  • Ensure full capacity is utilised.

Person Specification:

  • Experienced Co-ordinator / scheduler doing coordination and rota management
  • We are looking for someone who is vibrant, self‑motivated and passionate about making a difference to the lives of vulnerable people
  • Drive and Resilience
  • Excellent verbal and written communication skills, you’ll be comfortable on the phone as you are writing emails
  • Experience of working in a faced paced environment and flexible
  • Strong attention to detail and be a completer/finisher
  • A team player with strong interpersonal skills
  • Holding a full driving licence and access to your own vehicle.
