Abbeyview Surgery is a semi-rural, part dispensing Practice serving circa 9300 patients in Crowland, Lincolnshire. We have a strong team ethos where all departments work collaboratively to provide the best patient centred care possible.

Main duties of the job:

  • The position may be full time or part time for the right applicant.

About us:

Abbeyview Surgery is situated in the semi rural town of Crowland serving circa 9300 patients. This is an exciting opportunity to join our friendly team of dedicated professionals to deliver high quality, patient centred care to our patients. We work as a team, and with our PPG to identify ways to improve services for patients of the Practice.

Job responsibilities:

  • Provide continuing direct and indirect care to patients across the practice population.
  • Provide clinical observations, phlebotomy, flu vaccinations, ECGs, BPs and venepuncture, and other designated clinical tasks as required.
  • Assisting in wound care management following patient care plans.
  • Monitoring and support of the management of patients with long-term conditions / chronic diseases, using a holistic patient centred approach.
  • Support patients to improve and maintain their mental and physical health and wellbeing.
  • Assist with the co-ordination of Infection Control policy and audit.
  • Chaperone patients when required.
  • Support registered nurses to enable them to focus on the more complex clinical care.
  • Contribute to the delivery of integrated care.
  • Promote health and well-being to all patients, for example undertaking the NHS health checks, Hypertension reviews, Prediabetes checks.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services.

Person Specification:

  • Experience: Qualified Nursing Associate.
  • Previous experience of working in General Practice.
  • Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
