Abbeyview Surgery is a semi-rural, part dispensing Practice serving circa 9300 patients in Crowland, Lincolnshire. We have a strong team ethos where all departments work collaboratively to provide the best patient centred care possible.
Main duties of the job:
- The position may be full time or part time for the right applicant.
- We offer 6 weeks holiday plus Bank Holidays (pro rata), NHS Pension Scheme, free parking - and plenty of tea and coffee.
- Ongoing training is supported and encouraged by the Practice.
Job responsibilities:
- Provide continuing direct and indirect care to patients across the practice population.
- Provide clinical observations, phlebotomy, flu vaccinations, ECGs, BPs and venepuncture, and other designated clinical tasks as required.
- Assisting in wound care management following patient care plans.
- Monitoring and support of the management of patients with long-term conditions/chronic diseases, using a holistic patient centred approach.
- Support patients to improve and maintain their mental and physical health and wellbeing.
- Assist with the co-ordination of Infection Control policy and audit.
- Chaperone patients when required.
- Support registered nurses to enable them to focus on the more complex clinical care.
- Contribute to the delivery of integrated care.
- Promote health and well-being to all patients, for example undertaking the NHS health checks, Hypertension reviews, Prediabetes checks.
- Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to personalised care colleagues and local community and voluntary sector services.
Person Specification:
- Experienced HCA with experience of working in primary care.
- This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
