Main duties of the job:
You will work in a supportive, friendly environment with our integrated team of Doctors, ANP's, Clinical Pharmacist, Paramedic, Social Prescriber, Care Co-ordinator, Mental Health Nurse, Specialist Diabetes Nurse and Nursing team. The whole team works together to provide an innovative and caring service where patients are truly at the heart of everything we do.

Abbeyview Surgery is a semi-rural, part dispensing Practice serving circa 9300 patients in Crowland, Lincolnshire. We have a strong team ethos where all departments work collaboratively to provide the best patient centred care possible.
Job responsibilities:
- Supporting patient care: Collaborating with clinical teams to provide best practices and mutual support.
- Medication reviews: Conducting clinical medication reviews to manage patients with complex polypharmacy and chronic conditions.
- Patient education: Providing clinical expertise and educational resources on medications and treatment pathways.
- Administrative support: Managing prescription queries, conducting audits, and ensuring safe prescribing practices.
- Clinical leadership: Overseeing medicines optimization and quality improvement initiatives within the practice.
This role is pivotal for enhancing patient outcomes and operational efficiencies in general practice settings.
Person Specification:
Experience: Experience of working in a GP practice preferred.
Qualifications: Masters in Pharmacy. Need to have completed Primary Care Pharmacy Education Pathway. Prescriber preferred, but not essential.
