Active Living is a unique, award-winning Exercise Referral scheme designed to support people to increase their physical activity levels and improve their health. This initiative is a partnership between Abbeycroft, West Suffolk NHS Foundation Trust and Allied Health Professionals. Experience instructors will support patients through a programme of activity lasting either 12 weeks or 24 weeks, with follow-on options available for everyone to promote long term behaviour change. Our person-centred approach will ensure all aspects of health relevant to the client are considered and supported.
As a Health and Wellbeing Practitioner you will coordinate the Active Living scheme services and support the delivery of required sessions/classes. You will support and empower the client to use their skills and networks to overcome barriers and make positive changes to improve their health and wellbeing. You will also support clients with more complex needs, which may include clients with various long term health conditions and/or issues with accessibility. Your main area of responsibility will be to coordinate our Exercise on Referral scheme and relevant services, leading the management and where required lead on instruction and delivery.
Your responsibilities will include:
- Coordinate the Active Living and Exercise on Referral scheme and related services.
- Lead, where required, on instruction and delivery of sessions/classes.
- Carry out health and fitness assessments, consultations, and inductions.
- Develop sustainable physical activity plans to support lasting lifestyle changes.
- Monitor and report on key performance indicators and service delivery in line with organisational requirements.
Qualifications:
- Level 3 Fitness Qualification (minimum required).
- Level 3 Exercise Referral Qualification required.
- First Aid at Work certification.
- Relevant health coaching qualification or an accredited health coaching skills programme preferred.
- Candidates without a Level 3 fitness qualification will not be eligible for this role.
