CHAPS - Care Home Assistant Practitioner in Romford

Accountable To: Care Home Manager

Job purpose:

• To support the Unit Manager/Nurse in charge in the overall management of the unit/floor you are assigned to.
• To assist in the delivery of clinical care and personal care interventions, ensuring high standards of dignity, hygiene, and resident wellbeing.
• Assume responsibilities of the nurse-in-charge in their absence (if working on a nursing unit).
• To create an environment which respects the rights and wishes of the residents and encourages person-centred care and independence.

Knowledge, Skills, Abilities & Qualifications:

• CHAP qualification experience of working as CHAP for a minimum of 1 year.
• NVQ/SNVQ level 3 required with experience caring for the Service User group.
• Knowledge and experience of working in the care sector.

Principal Responsibilities:

Care Delivery & Personal Care:

• Support and deliver personal care interventions, including washing, dressing, bathing, continence support, oral hygiene, grooming, mobility assistance, and nutritional/hydration support, ensuring residents’ dignity is upheld at all times.
• Provide compassionate support with end-of-life personal care needs as part of the palliative care pathway.
• Promote independence wherever possible, enabling residents to participate actively in their own care.

Clinical Support:

• Work collaboratively with nurses to support clinical tasks within your competency, including:
• Medication Management and Administration
• Physical observations
• Urinalysis
• Skin inspection and simple wound dressings
• Aseptic technique
• Blood glucose monitoring and insulin administration
• PEG care and enteral feed administration
• Stoma care
• Venepuncture
• Catheter care and catheterisation (if trained and competent)
• Syringe driver checks (as delegated)
• Undertake and maintain medication competency in accordance with CQC/CI/NICE guidelines and home policy.

Care Planning & Documentation:

• Contribute to assessment, planning, implementation and evaluation of personalised care plans.
• Ensure systems are in place to encourage resident and family participation in their care planning and review process.
• Maintain accurate and timely records in accordance with regulatory standards and organisational policy.
• Report changes in condition promptly to the appropriate senior staff.

Leadership, Supervision & Teamwork:

• Support nurses in supervising care staff to ensure smooth running of shifts.
• Delegate tasks appropriately and monitor performance, escalating concerns as required.
• Participate in staff training and mentoring of junior colleagues.
• Promote a positive, inclusive and supportive team culture.
• Undertake supervisions of junior staff.

Quality, Safety & Compliance:

• Be fully conversant with and act in accordance with home policies, procedures and the regulatory framework (CQC/CI).
• Ensure safe working practices in line with Health and Safety at Work Act 1974.
• Demonstrate excellent standards of incident, accident and emergency response.
• Report safeguarding concerns (adult protection referral in Scotland), shortfalls in care or risks to the Home Manager without delay.
• Promote effective use of resources, ensuring adequate stock levels of equipment and supplies.

Communication & Professionalism:

• Liaise professionally with residents, families, visitors and external professionals.
• Provide clear, concise handovers and ensure continuity of care.
• Maintain confidentiality at all times in line with GDPR and home policies.
• Support the home’s reputation by promoting high standards of professionalism and conduct.

Organisational Responsibilities:

• Assist in resident’s admissions and discharges.
• Participate in audits and quality improvement activities.
• Support marketing and promotion of the home when required.
• Complete and pass the CHAP 6-week training programme, meeting all competencies.
• Attend mandatory, in-house and E-learning training as required.
• Undertake other duties appropriate to the role as directed.

Health and Safety:

• Be conversant with and ensure all staff are aware of the requirements under the Health and Safety at Work Act 1974 and of their compliance.
• Advise the Manager of any areas of concern.

Night Duty:

• This is a wakeful duty unless notified otherwise.
