Key Responsibilities- Delivering person-centred support that promotes independence and wellbeing.
- Assisting individuals with daily living activities and personal care where required.
- Supporting service users to participate in community activities, hobbies, and appointments.
- Encouraging choice, dignity, inclusion, and self-determination.
- Assisting with medication administration in line with service procedures.
- Maintaining accurate records and completing relevant documentation.
- Identifying and reporting safeguarding concerns appropriately.
- Building positive and professional relationships with service users, families, and multidisciplinary teams.
- Previous experience within health and social care is desirable but not essential.
- A compassionate and person-centred approach to support.
- Strong communication and interpersonal skills.
- Ability to work both independently and as part of a team.
