We are looking for an enthusiastic, highly motivated and committed individual with a wide range of NHS knowledge, a high level of awareness and an understanding of the local health care services. As a dynamic and forward-thinking Emergency Department, we are committed to providing excellent healthcare services to our local community. We are always looking at ways of improving our service.

The main focus of this role is information delivery, support and guidance to patients accessing Health Care services in Hackney. Patient navigation is poised to become a household word over the next decade and will help improve quality of care while driving more rapid movement through the healthcare system.

This post will improve services to patients attending the A&E Department and other centres within Hackney by raising the profile of community and primary care services and ensuring patients understand the value of being registered with a local GP for their ongoing primary health care needs.
