The predicted salary is between 24000 - 36000 £ per year.

To provide high quality, individualised outcome focused care and support to service users with learning disabilities and/or autism in a complex needs environment. To enable service users to improve their quality of life, including their mental and physical wellbeing, and supporting them to develop their skills and be as independent and in control of their life choices as possible.

ACCOUNTABILITIES & RESPONSIBILITIES

  • To provide person centred care and/or support for service users to the required standard in areas including, but not limited to:
  • personal care;
  • Medication management;
  • maximising physical and mental wellbeing and personal safety;
  • participating in the local community; promoting positive participation and inclusion at all times;
  • household living skills and money management;
  • identifying and effectively supporting plans to assist in reaching goals and aspirations.
  • To treat service users with dignity and respect; communicating effectively to identify and meet their diverse needs.
  • To develop person-centred support and risk management plans incorporating needs, goals, and aspirations with agreed outcomes.
  • To take appropriate action to ensure compliance with all statutory requirements, performance standards and our policies, practices, values and behaviours.
  • To maintain a clean, safe and secure environment in line with the required fire safety protocols, health and safety standards and lone working procedures.
  • To ensure that our and Local Authority Safeguarding policies and procedures are followed and managed effectively in order to promote the wellbeing of all.
  • To work in partnership with our other services, professional agencies and stakeholders, as needed in order to achieve a high quality service.
  • To work with our Living Positive Behaviour Support (PBS) lead and act as a mentor within the team for PBS interventions.
  • To act as designated shift leader in the absence of management.
  • To contribute to the effectiveness of the team and the service through effective team working and communication.
  • To attend and represent us at care plan review.
  • To act as a scheme champion for dignity and person-centred support. Where appropriate takes responsibility for taking an overview of support delivery for designated clients.
  • To undertake other duties as allocated by the manager.
  • TECHNICAL COMPETENCIES

    Education & Qualifications

    • NVQ 2/3 Diploma in Health and Social Care or willing to work towards.

    Knowledge & Experience

    • Knowledge/awareness of the needs of the specific client group and the complexities of care.
    • Knowledge of practice issues relating to the specialist area of service.
    • Awareness of health and safety requirements including guidance for lone working and fire safety (full training given).
    • Knowledge of benefits/entitlements system or ability to learn.

    Skills & Abilities

    • Strong interpersonal skills; sufficient numeracy and literacy to complete reports, log incidents and produce clear and precise support and risk management plans.
    • Sufficient IT skills to be able to update records on organisational database.
    • Able to demonstrate a person centred approach when working with individuals and groups.
    • Able to work in partnership with residents and other professionals.
    • Able to make effective crisis management interventions.
    • Able to work flexibly in line with staffing requirements that affect rota and location; able to work effectively as part of a team.
