The Reablement Social Worker will be part of a multi-disciplinary team comprised of team leaders, home support workers and case managers, working together with Pharmacists and Therapists to improve the independence of people going through a period of Reablement in Islington.
The role will include:
- Guiding people with more complex needs through the up to six week intervention.
- Coordinating a person's Reablement journey to maximise independence and meet Reablement goals.
- Completing social work specific activities and assessments in line with Safeguarding Adults procedures and the Mental Capacity Act.
- Reporting to the Reablement Manager for all operational activities and receiving formal supervision from the Senior Practitioner Social Worker.
- Working in a multi-agency context, sharing information, joint assessments and care planning.
- Promoting and safeguarding the welfare of vulnerable adults.
- Caring for a caseload of varied complexity and taking on care coordination responsibility.
- Undertaking Mental Capacity Assessments in accordance with the Mental Capacity Act 2005.
- Ensuring that the Care Act 2014 is implemented in all aspects of the role.
- Minimising and managing risk while enabling service users to maintain their chosen lifestyle.
- Obtaining informed consent for all care coordinated cases.
- Keeping the Team Manager informed of any significant events relating to the service.
- Providing advice and consultancy to colleagues and stakeholders.
- Keeping records of work undertaken and entering data on the database in accordance with Council Policy.
- Attending and contributing to multi-disciplinary and other meetings as required.
- Meeting contractual agreements by attending regular supervision.
- Using information technology systems efficiently and effectively.
- Achieving agreed service outcomes and personal appraisal targets.
- Undertaking training and participating in meetings and seminars for effective development.
- Carrying out duties in accordance with the Council's commitment to customer service excellence.
- Complying with the Data Protection Act and other legislation.
- Following the Council's Health and Safety Policy and relevant legislation.
- Investigating and managing risk according to Pan London Safeguarding Adults procedures.
- Providing concise, informative written and verbal reports based on analysis of complex evidence.
- Using a variety of social work methods to remedy personal and social difficulties experienced by clients.
