The predicted salary is between 24000 - 36000 Β£ per year.

Days, Evenings and Sleep in’s (7.30am – 3pm, 2.30pm – 10pm, 11pm – 7.30am)

Participate in the day to day delivery of a range of services for people with learning disabilities, physical disabilities, autism or associated mental health conditions; working with service users in a person centred way to promote greater choice, skill development, independence and inclusion.

Work in a variety of community-based settings, including day service activities, supported accommodation and short breaks.

Contribute to and participate in individual assessment, planning and review processes to ensure that services meet individual needs, choices and personal outcomes.

Contribute to and participate in risk assessment processes.

Work with all relevant parties to maintain an environment which is safe and conducive to the overall development and wellbeing of service users.

Requirements

  • Level 2 Diploma in Health and Social Care
  • Experience of working with vulnerable people in community or residential based settings
  • Understanding of individual assessment, care planning and review processes in care/ support settings
  • Adult DBS registered to the update service
