The predicted salary is between 17 - 22 £ per hour.

You will act as the first point of contact for residents and carers, providing high-quality information, advice, and initial conversations using a strengths-based approach. You will carry out non-complex assessments, act as a Trusted Assessor, recommend Telecare and simple equipment, and develop outcome-focused support plans to promote independence and wellbeing.

Key responsibilities

  • Handle enquiries and initial conversations for Adult Social Care
  • Complete strengths-based assessments and outcome-focused support plans
  • Provide information, advice, and signposting to relevant services
  • Identify safeguarding concerns and escalate appropriately
  • Recommend and arrange Telecare and simple equipment
  • Maintain accurate records and contribute to service improvement

Requirements

  • Experience in a customer-facing role, ideally within Adult Social Care
  • Knowledge of the Care Act, Mental Capacity Act, and safeguarding
  • Strong communication and assessment skills
  • Good IT skills and experience handling confidential information
