We are seeking a Brokerage & Placements Support Officer to join our team on a temporary basis in Buckinghamshire. In this role, you will arrange and coordinate care and support services for adults (18+) who are eligible under:
- The Care Act
- The Mental Health Act
- NHS Continuing Healthcare (including end-of-life care)
You will support individuals transitioning from hospital or short-term care settings through the ICB, such as rehabilitation services, ensuring they receive timely and appropriate support.
Key Responsibilities
- Spot-purchase care and support services to meet individual needs
- Arrange residential nursing placements, day care, transport, and other required services
- Coordinate care under NHS or Care Act requirements
- Facilitate hospital discharge and smooth transitions into community care
- Use multiple systems and follow several processes to ensure accurate placement activity
- Work collaboratively with internal teams, healthcare partners, and providers
About You
- Strong organisational and multitasking skills
- Experience in adult social care, health services, or brokerage (preferred)
- Confident using multiple digital systems
- Clear communicator with strong partnership-working skills
- Committed to supporting positive outcomes for adults in need
