Adult Social Care Worker Bromsgrove, Worcestershire 04/08/2025 β 3 months initially Monday to Friday, 9am β 5pm, 37 hrs/week Support the duties of the social work team. Work with adults, using a strength-based approach, who may have a wide range of care and support needs and require support to reduce and manage identified risks. Aim to prevent or delay individuals from developing longer term care and support needs. This enables individuals to be healthy for as long as possible, self-managing long-term conditions. Assess needs and translating these into person centred outcomes. Develop and implement care and support plans, and evaluating outcomes. Promote their well-being and to maximise independence and access to community resources. Work in partnership and engage other professionals to ensure there is a multi-agency approach. Identify adults and carers who may be at risk of abuse and neglect and support safeguarding procedures, alongside a qualified social worker. Attend meetings and reviews where necessary.
