Qualifications
- Experience in Case Management and Social Services
- Knowledge and experience in Elder Care and Medicine

- Excellent communication and organizational skills
- Ability to work independently and manage a team
- Relevant experience in a similar role is a plus
- Bachelor\’s degree in Social Work, Nursing, or a related field
