306 Medical Centre are looking for an individual to join our small and friendly team in the role as admin/care co-ordinator. This is a part-time position initially fixed for 1 year as a job share over specific days, initially Wednesday-Friday. We work together to deliver a high standard of care for our patients and are looking for a team player who will actively participate in promoting the care of our patients and who is able to communicate with the team in an effective and positive manner. The successful candidate will ideally have experience in a clinical or administrative role. Someone with an interest in Health and Social Care would be ideal. You should be organised, flexible and a good communicator. You should be motivated and a compassionate individual.
Main duties of the job
- Working alongside the Clinical Lead and Practice Manager to assist in implementing a full range of patient services within the practice.
- Managing your workload as delegated.
- Excellent verbal and written communication skills.
- Passion for helping people and providing personalised care.
- Strong leadership and management skills and the ability to motivate others.
- An understanding of the organisations policies and regulations.
- Ability to maintain a high level of confidentiality and respect for patients.
- Proficiency with computers and Microsoft office tools such as Word, PowerPoint and Excel, knowledge of Emis would be advantageous.
- Strong analytical skills and the ability to handle multiple tasks at a time.

Job responsibilities
- Your role would be to proactively identify (using population data) patients with long-term conditions and support their personalised care needs e.g. preparing for a shared decision-making conversation or providing holistic support.
- Help patients manage their needs through answering queries, making and managing appointments, and ensuring that good quality written or verbal information is available to help with choices about care.
- Assist with patient queries, signposting, facilitating access to self-management education and support for patients to take more control of their health/wellbeing.
- Provide coordination and navigation for people and their carers across health and care services, working closely with other primary care professionals.
- Promoting and supporting PCN health initiatives such as immunisations, health checks, smears.
- Proactively support call and recall process.
- Working proactively with the PCN pharmacist to support medication reviews.
- Excellent interpersonal skills and the ability to interact well with different kinds of people.
- Assisting with the Appointment Book.
- Provide administrative support to the practice.
- Working with the team to ensure that all clinical rooms are stocked and overseeing the ordering of items needed.

Person Specification
- Qualifications: GCSE grade A to C in English and Maths, A levels, NVQ Level 3 Health and Social Care.
- Experience in a healthcare setting.
- Knowledge of the EMIS clinical system.
- Disclosure and Barring Service Check: This post is subject to the Rehabilitation of Offenders Act (Exceptions Order) 1975 and as such it will be necessary for a submission for Disclosure to be made to the Disclosure and Barring Service (formerly known as CRB) to check for any previous criminal convictions.
