We are currently recruiting experienced Locum Social Workers to join a busy Hospital Discharge Team within West Lothian. This is an excellent opportunity for a confident practitioner who thrives in a fast‑paced, multidisciplinary environment and is committed to supporting safe, timely hospital discharges.

Key Responsibilities

  • Carry out strengths‑based assessments to support safe and appropriate discharge planning
  • Work collaboratively with NHS colleagues, occupational therapists, physiotherapists, and community teams
  • Complete Mental Capacity assessments and Best Interest decisions where required
  • Develop personalised care and support plans in line with assessed needs
  • Ensure accurate, timely case recording and maintain statutory responsibilities
  • Apply relevant adult social care legislation, including the Adult Support and Protection (Scotland) Act, the Social Work (Scotland) Act, and the Mental Health (Care and Treatment) (Scotland) Act

Requirements

  • Degree / Diploma in Social Work
  • SSSC registration
  • Previous experience within hospital discharge, adult services, or a fast‑paced statutory team
  • Strong risk assessment and decision‑making skills
  • Ability to work autonomously and effectively as part of a multidisciplinary team
  • PVG membership
