Are you passionate about making a real difference in people's lives? 21HSC are recruiting on behalf of Care Support Scotland who support people with a diverse range of care needs to live fulfilling, independent lives within their communities. The people they support include people with learning disabilities, autism, mental health conditions, acquired brain injuries, and mobility issues. They provide different levels of care tailored to each individual's needs, goals, and aspirations.

As a Support Worker, you will play a key role in delivering person-centred, high-quality support that promotes wellbeing, independence, dignity, and inclusion.

Key Roles and Responsibilities:

  • Deliver high-quality, person-centred support tailored to the individual needs, goals, and aspirations of the people you support.
  • Support individuals with daily living, personal care, tenancy sustainment, emotional wellbeing, and social inclusion where required.
  • Contribute to and implement personalised support plans, participating in regular reviews, goal setting, and risk assessments.
  • Enable individuals to access educational, employment, housing, health, and recreational opportunities within their communities.
  • Support people to develop life skills and manage their home environment to promote independence.
  • Prompt or assist with medication in line with prescribed instructions and organisational policies.
  • Promote health awareness, positive lifestyle choices, and emotional wellbeing.
  • Maintain accurate, respectful, and professional records using electronic and written systems.
  • Participate in key working responsibilities and contribute to positive outcomes for the people you support.
  • Work collaboratively with colleagues, families, advocates, and external agencies to ensure effective support delivery.
  • Ensure all work is delivered in line with risk assessments, health & safety requirements, and organisational procedures.
  • Uphold confidentiality, dignity, equality, diversity, and inclusion in all aspects of support.
  • Engage in continuous professional development and attend mandatory training to develop specialist skills and knowledge.
  • Follow the National Care Standards and Scottish Social Services Council (SSSC) Codes of Practice.

Essential Skills and Attributes:

  • A genuine passion for supporting people to achieve positive outcomes and live independently.
  • Experience working independently and as part of a team in a care, support, or voluntary setting.
  • Strong communication and relationship-building skills.
  • Ability to adapt to changing needs and respond positively to challenges.
  • Good organisational skills with the ability to manage workload effectively.
  • Basic IT skills, including confidence using electronic systems and Microsoft applications.
  • Ability to maintain clear, accurate, and professional records.
  • Commitment to promoting equality, diversity, dignity, and inclusion.
  • Ability to read, write, and communicate effectively in English.

Desirable:

  • Previous experience within an adult social care setting.
  • Understanding of learning disabilities, autism, mental health conditions, acquired brain injuries, or mobility support needs.
  • Knowledge of the Care Inspectorate, SSSC standards, and person-centred approaches to care.
  • Experience of writing reports, support notes, or care plans.
  • Positive Behaviour Support or Trauma-Informed Practice experience or qualifications.

Qualifications:

A practice qualification recognised by the SSSC (minimum SCQF Level 6) in Housing Support Services / Care at Home, or willingness to work towards this qualification.
