Key Roles and Responsibilities:
- Deliver high-quality, person-centred support tailored to individual needs and aspirations.
- Utilise case management systems to review, monitor, and evaluate progress.
- Facilitate key work sessions, reviews, and meetings in line with support plans.
- Provide advocacy, advice, and support to ensure service users meet occupancy conditions.
- Work in partnership with external agencies to maximise opportunities for supported individuals.
- Manage a personal caseload while contributing to the wider team.
- Support referrals, admissions, and discharges in accordance with procedures.
- Maintain a safe environment through regular risk assessments, health & safety checks, and cleaning duties.
- Help individuals with tenancy management, budgeting, and accessing services.
- Uphold data protection and confidentiality requirements while maintaining accurate records.
- Play an active role in service development, professional learning, and continuous improvement.
Essential Skills and Attributes:
