Community and Transition Support Worker

Location:Edinburgh

Contract Type:Permanent- full time

Salary:£26,318

As a Community and Transition Team Support Worker you will work in partnership to provide practical, emotional, and social support to care-experienced and vulnerable children, young people (aged 1626), and their families. Helping them stay safe, access housing and services, build strong relationships, and reach their full potential.

Key Roles and Responsibilities:

  • Provide intensive one-to-one support to young people and young parents, helping them build life skills, emotional resilience, and independence.
  • Support young people in residential, supported, or independent housing, helping them manage tenancies and transition into less supported environments.
  • Develop and implement individual support plans, tailored to each young persons needs and goals.
  • Facilitate group work and engagement activities that promote participation, connection, and wellbeing.
  • Strengthen parent-child attachments and support healthy, confident parenting.
  • Promote community integration, connecting young people with services, opportunities, and support networks.
  • Assist with practical issues like accessing benefits, gaining employment, securing accommodation, and improving income.
  • Provide high-quality case management, including risk assessment, progress reviews, accurate record-keeping, and multi-agency collaboration.
  • Represent the Trust in external meetings, such as case reviews and legal proceedings.
  • Maintain professional standards, safeguarding responsibilities, and compliance with relevant policies and regulations.
  • Be flexible in working hours to meet the evolving needs of young people.
  • Contribute to service development through feedback, evaluation, and involvement in team activities and continuous improvement.
Essential/Desirable Skills and Attributes:
  • Skilled at building trusting relationships with young people.
  • Deeply aware of the complex challenges facing those leaving care.
  • Knowledgeable in safeguarding legislation and good practice.
  • Experienced in collaborating with partner agencies and advocating for young peoples rights.
  • Capable of motivating and mentoring young people toward positive outcomes.
  • Strong in communication, organisation, and report writing.
  • Reflective and open to feedback and continuous learning.

Qualifications & Experience:

  • A degree-level qualification in a relevant field or substantial equivalent experience is essential.
  • Experience supporting care-experienced young people, particularly those transitioning out of care, is highly desirable.
