About the Role:
As a Care Co-ordinator, you will play an integral part in managing the daily coordination of the operations. This role is extremely fast-paced and requires someone truly dedicated and able to remain positive and professional when working under pressure. Overseen by the Care Manager, you will be responsible for planning and coordinating care staff schedules on a daily basis. You will also be responsible for liaising with clients, their families, and other professionals to ensure that the care needs of clients are being met, as well as accepting new packages of care.
Main Duties:
- Work alongside office team to ensure sufficient cover to deliver all of the care and support plans on time.
- Manage effective coordination of scheduled hours and staff rotas.
- Identify and match care staff with the right skills and experience to deliver each client's care and support plan.
- Manage staff holidays and staff absences.
- Manage supervisions and appraisals of care staff.
- Manage unexpected emergencies and changes in care packages.
- Make sure all relevant information in care plans are current and up to date for clients' needs.
- Manage continuity of care for all clients.
- Liase with recruitment officer to ensure there is sufficient care staff with the correct skill set available to meet the needs of the business.
- Manage on-call if necessary.
Skills and Qualifications:
- An experienced care coordinator with an excellent telephone manner and good communication skills - 12 months experience needed.
- You will need to be customer-focused and must be able to multi-task and work well under tight deadlines and pressure.
- Either SVQ3 qualification or must be willing to complete the course.
- Driving licence (preferred).
- Have the ability to support clients to enjoy the best quality of life they can in the comfort of their own homes.
- Being of a compassionate and supportive nature, along with excellent organisational skills and working in a respectful and dignified manner.
