We are currently seeking Support Workers (Band 34) with experience in the following areas:
- Adult Disability Supported Living
- Autism Outreach

Responsibilities
- Supporting individuals with daily living activities
- Encouraging independence and community participation
- Delivering person-centred care in line with individual support plans
- Working collaboratively with multidisciplinary teams
