Adults Social Worker – Hospital Discharge – Basingstoke Hospital An exciting opportunity has arisen for a Qualified Social Worker's to join our friendly and supportive hospital discharge team at Basingstoke Hospital. Responsibilities: Working with the acute team. The focus will be on supporting the turn around of patients at the front door and working with the Single Point of access here at the hospital. This role requires you to be on site 5 days a week as you will be assessing patients for discharge planning.Our Social Workers are an asset and are encouraged to be creative and autonomous, while enabling you to work flexibly. Hampshire are committed to providing regular, reflective and effective supervision to support you, while ensuring you also have the working conditions you need for good practice to flourish.
