Job Description: We are seeking a compassionate and dedicated Per Diem/PRN Social Worker to join our team at the Southern Utah Veterans Home. In this role, you will be responsible for providing social work services to our residents and their families, with a focus on promoting their emotional and social well-being.

Responsibilities:

  • Conduct psychosocial assessments of residents to identify their needs and develop individualized care plans
  • Provide counseling and support to residents and their families to address emotional and social issues
  • Coordinate with interdisciplinary team members to ensure comprehensive care for residents
  • Advocate for residents\’ rights and assist them in accessing community resources
  • Participate in care planning meetings and provide input on residents\’ psychosocial needs

Qualifications:

  • Master\’s degree in Social Work from an accredited program
  • Licensed Social Worker in the state of Utah
  • Experience working with older adults and/or veterans preferred
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
