Job Opportunity: Clinical Pharmacist in General Practice Are you a dedicated pharmacist seeking an exciting role within a dynamic general practice team? We are looking for a passionate Clinical Pharmacist to provide patient-focused care, enhance medication safety, and improve healthcare outcomes. About the Role: As a Clinical Pharmacist, you will work collaboratively with GPs, Practice Managers, and a multi-disciplinary team. This patient-facing role involves providing expert clinical medicines advice, managing prescription queries, and optimizing medication use. You will play a pivotal role in chronic disease management, medication reviews, and the development of safer prescribing systems. Key Responsibilities: Clinical Medication Reviews: Conduct medication reviews, especially for patients with complex polypharmacy, and recommend improvements to GPs. Long-term Condition Clinics: Support patients with conditions like asthma and COPD through medication optimization and adherence support. Care Home & Domiciliary Reviews: Ensure safe and effective prescribing for care home residents and patients at home. Repeat Prescribing: Manage repeat prescription systems, ensuring appropriate monitoring and safety checks. Medicines Reconciliation: Oversee medication adjustments post-hospital discharge to ensure continuity of care. Patient Support: Address medication-related queries via in-person clinics and a telephone helpline.
