The Role

We are looking for a dynamic and committed individual to join our Via Harrow service in the borough of Harrow.

This exciting and rewarding role will carry a restricted caseload while providing you with an intensive training programme for growth and innovation within a supportive and friendly multidisciplinary team of clinical and non-clinical professionals.

The main purpose of this role is to provide you with the right skills and knowledge which will enable you to move towards being able to support clients with a range of treatment needs, including Needs Assessment, Care planning & Risk Management, Harm reduction, Safe prescribing, Group work and Safeguarding. Responsibilities will include the following.

  • Working as part of a dynamic team delivering an effective and safe service covering Duty, Referrals and Assessments.
  • You will be responsible for managing a caseload of clients which include opiate substitute prescribing, relapse management, providing a good quality of interventions at varying stages of their recovery.
  • Work as part of a creative team in developing and delivering a wide range of group sessions.
  • Liaising with external agencies (GP, CMHT, Hospitals, Safeguarding Teams) to promote safe treatment and wellbeing (work in partnership approach) for the best outcome for all clients.
  • The role involves outreach (based out of the service) such as to the local CMHT (Community Mental Health Team), attending team meetings, and working together to increase numbers of referrals from services within the Borough of Harrow
  • Duties may include supporting other areas of service delivery as well as helping to take part in a wide range of special interests and activities that support health and wellbeing such as smoking cessation and local events.

The Service

Via Harrow is an established Community Integrated service that is well performing and highly functioning. The service consists of both clinical and psychosocial and is CQC registered to provide a range of clinical interventions including BBV & health interventions, OST, and community detox, as well as robust family and carers and service user involvement provision.
