Newbirth Care is seeking a proactive, experienced Agency Social Worker to join a Urgent and Emergency Care Team, working across acute hospital settings such as the Emergency Department (ED), Acute Medical Unit (AMU), and Same Day Emergency Care (SDEC). This is a fast-paced role focused on delivering high-quality statutory social work services to support individuals at the point of hospital attendance or admission to discharge.

You will be working collaboratively as part of a team to ensure timely assessments, appropriate crisis interventions, safeguarding actions, and effective discharge planning, helping to reduce delays and promote independence.

Key Responsibilities:
Undertake proportionate Care Act assessments, mental capacity assessments, and discharge planning within emergency and urgent care environments.

Provide responsive crisis interventions and practical support to individuals in acute distress or at risk of harm.

Liaise with health professionals, family members, and partner agencies to ensure holistic and person-centred care planning.

Identify and escalate safeguarding concerns and complete safeguarding enquiries under Section 42 of the Care Act 2014.

Contribute to multi-agency meetings, including hospital flow meetings and discharge planning boards.

Maintain accurate case notes, documentation, and records in line with the local authority\’s systems and legal obligations.

Ensure timely and appropriate use of community-based care and support services to avoid unnecessary hospital stays.

Support strength-based practice and independence-focused discharge pathways.

Participate in team handovers, reflective sessions, and case discussions as required.

Requirements:
Essential:

Qualified Social Worker with current registration (Social Work England).

Minimum of 2 years’ experience in adult social care.

Proven experience of hospital discharge work or urgent/emergency care social work.

Sound knowledge of the Care Act 2014, Mental Capacity Act 2005, and other relevant legislation.

Confidence in managing complex cases and working under time pressure.

Excellent communication and organisational skills.

Ability to work both independently and within multi-disciplinary teams.

Enhanced DBS on the Update Service (or willingness to obtain one).

Desirable:

Experience working in local authority hospital discharge or integrated teams.

Familiarity with [insert local case management system – e.g., Mosaic, Liquidlogic, LAS].

Best Interests Assessor or AMHP qualification (advantageous but not essential).
