About the job
As a Specialist Women’s Case Co-ordinator, you’ll play a vital role within Cardiff’s Homelessness Multi-Disciplinary Team (MDT) — a collaborative network of professionals delivering holistic, person-centred support to individuals experiencing homelessness.
In this role, you will:
Deliver assertive, needs-led outreach and support to women sleeping rough or facing entrenched homelessness, with a focus on long-term engagement and harm reduction.
Work closely with MDT partners — including housing officers, social workers, health professionals, and support agencies — to build integrated and coordinated support pathways.
Manage a caseload of women with complex needs, carrying out in-depth assessments and developing personalised support plans tailored to each individual’s circumstances.
Respond to a range of overlapping issues such as domestic abuse, sexual violence, exploitation, offending behaviour, and substance misuse, using a trauma-informed approach to provide emotional support and practical interventions.
Advocate on behalf of the women you support in multi-agency settings, including MARAC and SWOT meetings, ensuring their voices are represented and their needs are prioritised in decision-making.
Support access to appropriate accommodation, accompany clients to appointments, and help them engage with specialist services to improve their safety, wellbeing, and long-term outcomes.
Contribute to the strategic and operational development of the MDT by participating in relevant strategy groups, women-specific forums, and sector conferences — feeding back learning and best practice to enhance the service as a whole.

While proven experience working with individuals with complex behaviours and a solid understanding of the challenges faced by homeless women is highly desirable, we also value empathy, resilience, and a strong commitment to person-centred support.
The role operates in a demanding and often fast-paced environment, so the ability to remain calm under pressure, adapt to changing situations, and use sound judgement is essential. You will be expected to work collaboratively within the homelessness multidisciplinary team and across partner organisations, contributing positively to a culture of respect, inclusion, and continuous learning.
Additionally, the successful candidate will support the management team by assisting with the day-to-day running of the service, contributing to service development initiatives, and supporting other staff members as needed. Strong organisational skills, effective communication, and a proactive approach to problem-solving will be key to thriving in this role.
