We focus on what matters most to individuals and their carers, helping them achieve their goals, stay connected to their communities, and live fulfilling lives About the role You\’ll ensure safe and timely discharges from hospital by: Assessing the needs of patients and carers Arranging appropriate care packages Liaising with professionals and agencies Supporting person-centred discharge planning We have posts available in: Barnet & Chase Farm Hospital – within ULEZ zone Watford General Hospital Herts Valleys Community Hospital Team – 24 hours per week East and North Hertfordshire Transfer of Care Team We offer a 7 day service and currently operate a 1-weekend-in-8 rota across the Hospital Team About you Essential : • Social Work degree • Registration with Social Work England • Driving licence
