An opportunity has arisen in the Performance Management Department for an MDT Coordinator/Patient Pathway Tracker. The team requires an enthusiastic team member who is committed to having a positive impact on cancer patient pathways. You will have had experience working within the NHS preferably within a cancer data environment. An understanding of current cancer pathways and MDT processes would be extremely desirable. You will be required to work flexibly to meet the needs of the service.

Candidates must have Microsoft Office Suite experience. You will have good organisational skills and be able to communicate confidently with all levels of staff on a wide range of information

As an MDT Co-Ordinator/Patient Pathway Tracker you will be responsible for completing, but not limited to, tasks such as:

Tracking target patients in a timely manner from referral to first treatment. Attending regular meetings and provide updates on the patient pathway. Accurately maintain a patient tracking list. Liaise with other trusts.

Being responsible for the facilitation of Multi-disciplinary team meetings ensuring MDT meetings run effectively and are well structured.

Accurately prepare and circulate lists of patients for discussion at each multi-disciplinary team meeting (MDT) using information provided by secretaries, lead clinicians and nurse specialists.

To attend the multi-disciplinary team meetings, keeping comprehensive records of attendance and outcomes of the meeting, and distributing to relevant team members. To ensure timely and accurate input of the outcomes of the MDT meetings onto local patient administration system, CWP.

Facilitation of multi-disciplinary team meetings to ensure the meetings run effectively and are well co-ordinated and all relevant patients are discussed.
Liaise with clinical and administrative staff to create lists for circulation prior to the MDT meetings
Attend and facilitate each MDT, and use the video conferencing equipment, as needed.
Distribute outcomes to all concerned following the meetings and ensure that this information is correct and complete and recorded on CWP.
Report the current position of the patient on their cancer pathway to clinicians during meetings so that they can make clinical decisions based on this information.
To support the teams in the collection of evidence, documentation and/or data for the National Peer Review Programme.
Ensure that the MDT activities are recorded and the results documented, eg attendance records, highlighting to the Cancer Services Manager / Lead Nurse should any members attendance fall below the required standard.
Ensure that MDT members are kept up to date with the upcoming schedule of MDT meetings and are aware of any changes to date, venue etc.
Ensure appropriate cover at all MDT meetings during periods of sickness or annual leave

PATIENT PATHWAY TRACKING

Coordinate the administrative pathway of patients with a confirmed cancer to ensure that they meet National Cancer Waiting time standards.
To proactively track the cancer patients and escalate all potential delays to the Cancer Performance Manager/Divisional Managers according to the agreed escalation policy to ensure actions are taken to avoid any breaches .
Ensure up to date and relevant information/data is collected to enable the full and accurate production of the PTL (Patient tracking list) and attend weekly meetings to discuss
To maintain the systems and processes required to support the tracking of patients in their respective targets and identify service improvements where necessary
Provide detailed breach analysis reports within 5 days of the date of the first definitive treatment date (FDT) for any 62 day patients with waiting times outside of national standards
To liaise with colleagues from other Trusts to ensure that data is accurate for patients transferred from other hospitals to undergo treatment.
To validate all treatments and pathways within one working day of the treatment date
To ensure that exceptions and alterations to the waiting times of individual patients are appropriately supported by documentation and can be audited

Maintain the generic 62 day CaRPs inbox, processing and registering referrals for patients on a 62 day pathway in accordance with the SOP
