Support residents to live more independently, with compassion at the heart of your workAs a Social Worker in Adult Social Care, you\’ll work directly with people in Kensington and Chelsea who need support to live well and safely. From your first conversation to the long-term plans you help shape, you\’ll be building relationships that empower individuals, reduce risks, and make a lasting difference.Working Style:You\’ll be based in the Borough for 3 days a week, playing a key role in serving the local community. Whether you\’re working on the frontline or behind the scenes, you\’ll be part of a dedicated team making a real difference to residents\’ lives.What you\’ll be doing:Each day will bring variety and purpose. You\’ll begin by reviewing referrals, prioritising cases, and planning your visits – from meeting residents in their homes to working alongside health and care professionals across the borough.Your focus will be on assessing needs, creating person-centred care plans, and arranging the right services that enable choice, dignity, and independence. You\’ll also act as an advocate, ensuring that every resident you support has a voice in decisions about their life.Collaboration is key – you\’ll be working closely with NHS partners, voluntary sector teams, and other professionals to deliver joined-up care. You\’ll also contribute to safeguarding work, maintain clear records using digital systems, and stay current with legislation, local procedures, and best practice.This is a supportive and team-based role. You\’ll be confident in assessing needs, building rapport with residents and carers, and translating complex needs into clear support plans.A strong understanding of key legislation – including the Care Act, NHS, Community Care Act and Mental Capacity Act – is essential. But just as important are your values: empathy, respect, and a commitment to equity and inclusion in everything you do.You\’ll have excellent communication and IT skills, be confident in your judgement, and bring a calm, curious approach when situations are challenging.
