Adult’s Social Worker
Team: Hospital Discharge
Location: Manchester

Details:
We are looking for an experienced social worker to join the hospital discharge to assess team at Harpurhey District office in north Manchester. This is a hybrid role working within the Intermediate care unit/home and harpurhey district office. You will be experienced in completing strength based care act assessments, mca and bims. You must be able to work well with others and part of the wider MDT with health colleagues to help arrange timely discharge from the intermediate care unit in north Manchester. There will be some duty work and also weekend cover approx 1 in 5 with 2 days taken back in the week.

  • DIPSW or equivalent qualification in Social Work
  • Social Work England registration
  • Enhanced and in date DBS certificate.
  • Excellent communication skills and be able to make good decisions based on information available.
  • Can work alone and within/contribute to a team/multi-agency set ups
  • Possess knowledge of service needs.
  • Competent IT skills and be able to navigate electronic file/recording systems.
  • Can write clear, concise, and robust assessment/reports/files.
  • Have a flexible approach to work and working hours when the need arises
