Are you the special person who could fulfill the role of a Learning and Disabilities Practitioner, giving families a short break and providing learning opportunities and independence to young people aged 5 to 18 years with a disability or complex health needs?
This is a rewarding and exciting opportunity to join an experienced and dedicated team of Practitioners working across Lincolnshire providing the Children with Disabilities Support Service projects.
This role is based in Lincoln and the surrounding area and offers short breaks of community and youth activities for up to 3 hours in the evening after school, as well as holiday clubs for 5 hours during holiday periods.
You will work with a small, supportive team and be involved in planning and facilitating high-quality sessions that support young people\’s targets and increase their confidence. Our aim is to support all children and young people with a disability or complex health needs, working together to ensure they are happy, healthy, safe, and the best they can be.
To achieve this, you will provide children and young people aged 5-18 years with disabilities and their families with high-quality targeted support that promotes self-confidence and independence, working as part of an integrated team.
The Early Years Alliance will provide you with exciting training and opportunities to learn new skills to support your professional development.
Your responsibilities will include:
- Planning and facilitating community, youth, and holiday sessions in local venues, providing appropriate activities in a safe, welcoming environment that promote self-confidence and independence.
- Assessing, monitoring, and tracking children and young people\’s progress, particularly in relation to their Education, Health and Care Plan (EHCP), using tools like Tapestry and Outcome Wheels.
- Supporting parents and caregivers to access advice and support from the Local Offer, encouraging peer group participation, learning, and engagement.
Qualifications and experience required:
- NVQ Level 3 in Early Years/Childcare, NVQ 3 Youth/Playwork, or equivalent, or relevant experience working with children with disabilities or complex needs.
- Understanding of meeting the individual needs of disabled or complex health needs children and young people.
- Effective communication skills, both in person and in writing, with professionals, staff, and organizations.
- Experience working as part of an integrated team with social care, early help teams, educational providers, voluntary organizations, and other partners.
