Are you passionate about supporting people in your local community and working to achieve goals with our service users to live independently in their own home?

We are looking to recruit a Reablement Co-ordinator on a permanent basis on a -hour contract who truly care and are committed to making a difference in an individual’s life.

Being a Reablement Coordinator means supporting people in their own homes for a short period to assist them in reaching their full potential. This may include managing various aspects of daily living skills, setting clear and achievable goals to help people rebuild their independence and confidence to sustain independent living at home and in their communities.

You will effectively work within the reablement team to deliver a high-quality reablement service with the aim of promoting and sustaining people’s independence living in the community. Supervise staff and mentor the personal development of Reablement workers.

A driving licence is essential for this community-based role.

Complete service users\’ journeys well, ensuring coordination and compliance with organisational policies, procedures, and Care Quality Commission regulated guidance. To ensure that the reablement service achieves the best quality outcomes for service users, their carers, and families.
