As an In Community Navigator (ICN) you will support people with disability, their families and carers to access the NDIS, build local connections and navigate community supports. This highly visible, place based role is all about grassroots engagement, trusted relationships and creating inclusive pathways to empower people to live their best lives.

What you\’ll do:
- Guide individuals through the NDIS access process, helping them understand eligibility, gather documentation and connect them with appropriate supports.
- Deliver short term, community-based support to promote inclusion.
- Provide practical assistance to help people build confidence and connections while navigating the system.
- Attend community hubs, schools, health services and local events.
- Establish and maintain strong relationships with grassroots organisations, service providers and informal support networks to bridge service gaps.
- Facilitating direct connections between participants and community supports
- Maintaining accurate documentation and meeting performance targets
- Work closely with Local Area Coordinators, Team Leaders and other Feros Care staff to deliver participant focused services.
- Participate in team meetings, regional planning and initiatives that promote cultural safety, inclusion and continuous improvement.
What you\’ll bring:
- Experience in Community engagement, disability support or similar participant facing roles.
- Understanding of the NDIS participant pathway and scheme access processes
- Ability to build strong, trusted relationships through outreach and community participation.
- Strong communication, facilitation and organisational skills.
- Confidence working independently in community settings.
- Proficiency in data entry, reporting and case management systems.
- A commitment to inclusion, diversity and working in partnership with people with a disability.
Qualifications & Checks:
- Qualification in community services, disability, social work, or a related field (desirable).
- Current unrestricted driver\’s licence.
- Reliable transport to support outreach and in community work.
- Current NDIS Worker Screening Clearance & Working with Children Check (or willingness to obtain).
- Undergo a Nationally Coordinated Criminal History Check.
- Lived experience of disability is highly valued.
