Adult's SW - Reablement Team - Southwark

We are recruiting for a social worker within Intermediate Care Southwark. The team has two functions, including a Reablement Service and Urgent Response service. Both are multi-disciplinary teams with Occupational Therapists, Physiotherapists, Nurses, Pharmacists, Social Care staff and Rehab Support Workers and OT Assistant Practitioners. The post would be ideal for an experienced social worker who has experience of working in a Multi-Disciplinary Setting and working to short timescales, at times responding to crisis situations.
