Are you an experienced Healthcare Support Worker looking for a new challenge in a community setting? Join our Community Care Teams and help us provide high-quality, innovative care to the people of Harrogate and Rural areas in the comfort of their own homes.
As a Community Healthcare Support Worker with Harrogate District Foundation Trust, you will be based in one of our four Community Care Teams. The service operates from 7am to 8pm daily, with flexible shift patterns available.

Key Responsibilities
- Assess people\’s health and social care needs
- Take and interpret physical measurements and samples as delegated by the team
- Monitor changes in patients, interpret their significance, and inform the team of notable changes
- Assist in planning, delivering, and evaluating nursing care and rehabilitation programmes according to care plans
