This position offers a sign-on bonus and is located in Falmouth, MA, with a schedule of 37.5 hours per week, Monday through Friday, 8 am to 4 pm, including rotating weekends and holidays.
Responsibilities include:
- Maintaining annual HHRG values at the National Standard.
- Utilizing HHRG values to establish visit frequency and interdisciplinary utilization.
- Meeting National Benchmarks for annual outcomes.
- Referring to community resources as appropriate.
- Evaluating patient needs, developing and revising care plans, documenting accordingly.
- Participating in interdisciplinary case conferences and supervising Home Health Aides and COTAs.
- Providing patient and family education on occupational therapy treatments.
- Completing timely documentation and discharge planning.
- Adhering to departmental guidelines for supervision and case conferencing.
Qualifications:
- Bachelor\βs or Master\βs Degree in Occupational Therapy from an accredited institution.
- Current OT license in Massachusetts.
- Minimum one year of clinical experience and CPR certification.
- Preferred computer skills.
