About The Post

An exciting opportunity has arisen for a Short Breaks Family Support Worker:

This role involves working with children who have complex disabilities and their families, undertaking short break assessments, short break reviews, the development of short break packages, transition work for adult services, facilitating events & completing referrals to continuing health care. There may also be some direct family support work with families involved.

What you will need?

  • Experience of working with children and their families providing support to improve their situations.
  • Experience of working with colleagues across a range of agencies to support children’s outcomes
  • Level 2 qualifications in Maths & English or Level 2 Certificate for the Childrens and Young Peoples Workforce Social Care Pathway
  • ICT Training
