Advanced Clinical Practitioner – Community Specialist Palliative Care
Job Summary
This full‑time advanced clinical practitioner (ACP) role works within the Specialist Palliative Care (SPC) multidisciplinary team, supporting the SPC caseload across eight neighbourhood teams and partnering with hospice and hospital SPC services. The ACP provides clinical leadership, embeds general palliative care competencies across community services, and drives quality improvement, research and innovation. Key priorities include expanding non‑medical prescribing, championing patient safety, supporting autonomy in advanced clinical decision making and fostering collaboration with system partners.
Responsibilities
- Lead clinical workforce development and embed general palliative care expertise in community services.
- Design, implement and audit non‑medical prescribing protocols to improve symptom management.
- Support advanced clinical decision making, supervise and mentor junior staff, and promote autonomy in practice.
- Champion patient safety, continuous improvement and quality of care through clinical audit and service evaluation.
- Collaborate with multidisciplinary teams and external partners to advance palliative care standards.
- Drive research, innovation and evidence‑based practice improvements within the SPC service.
Qualifications
- Essential
- Registered General Nurse (RGN) or equivalent qualification.
- Degree (Bachelor’s) – preferred a Master’s in Advanced Practice, Palliative Care or equivalent.
- Non‑medical prescriber V300 certification.
- Relevant professional clinical and diagnostic training.
- Mentor qualification.
- Teaching qualification.
- Clinical supervisor qualification.
- Community nursing specialist qualification.
Knowledge, Skills & Experience
- Essential Knowledge
- Up‑to‑date awareness of national and local health and social care agendas.
- High level understanding of multi‑agency working, clinical governance and quality improvement.
- Strong communication skills, both written and verbal, including the ability to discuss difficult topics with patients and families.
- Experience in clinical audit, research and knowledge of audit / research methodology.
- Development and implementation of clinical protocols and guidelines.
- Experience with community development initiatives.
Professional Experience
- Essential Experience
- Extensive post‑qualification experience in specialist palliative care.
- Evidence of supervision experience, supporting trainee ACp and practice nurses.
- Proven ability to lead research, audit and quality improvement projects.
- Advanced communication skills and experience in multi‑agency leadership.
- Advanced communication training.
- Patient involvement/feedback leadership experience.
- Experience delivering nursing‑led services in primary or community settings.
- Recent experience at an advanced clinical level.

- Salary: £55,690 – £62,682 per annum (Band 8a).
- Contract: Permanent.
